Report of a Case of Superior Mesenteric Artery Syndrome , Associated with Acute Pancreatitis and Cholangitis , due to Intestinal Malrotation

Abstract

|
This report describes a 16 year old female who had superior mesenteric artery syndrome, associated with acute pancreatitis and cholangitis due to intestinal malrotation.
The patient, on the first admission, was medically treated as a simple acute pancreatitis with good result, but looked over the underlying causes by misinterpretation of the radiological findings, which had shown the abnormally situated small bowels to the right side of abdomen and with distended gaseous colon to the left side.
On the second admission, 6 month later of the first, she showed also the symptoms compatible to the recurrence of acute pancreatitis and cholangitis. But upper G-I series disclosed duodenal obstruction on its third portion and markedly dilated stomach and duodenum.
Under the diagnosis of superior mesenteric artery syndrome, operation was performed with good result and found as follows:
1) Duodenal obstruction on its third portion with dilated stomach and upper duodenum.
2) Diminished mesenteric pulsation, especially of superior mesenteric artery.
3) Intestinal malrotation.
4) Inflamed pancreas and biliary tract.
